Outage [Solved]
 
In the last 25 minutes we have experienced an outage. At this time everything is back up an stable, and we are investigating what happened on-site.

We have investigated this problem very thoroughly and we have found on that this was a power (electricity) related problem. This problem affected one of our racks, on which both powerfeeds (A and B) failed. This was caused by the breakers which both tripped.

All servers and equipment in this rack are connected to both power feeds (redundant), so if one power feed fails, the other power feed will simply take over (without interruption).

The problem was more than likely that the power supply of a switch within this rack short-circuited, and caused the A-feed to overload (and as such, the breaker tripped). Then all equipment automatically switched to the B-feed, on which the breaker then also tripped.

In this rack, we are also running an additional UPS, which was supposed to filter problems like these. We think that due to the short-circuit the battery of the UPS was drained very quickly, and as such, it had to switch back to the normal power source, causing the breakers to trip. For now this is just an assumption, we are investigating this matter with engineers of the supplier of the UPS (APC).

Again - it was not the UPS which caused this problem - it's just something that was supposed to prevent problems like these. The actual troublemaker appears to be a switch as stated above. This switch has now been replaced as a whole, and the new switch is also running from a completely separate power source. This should solve the problem permanently, however as said earlier, we will continue to investigate what happened with the UPS exactly, in order to avoid issues like these in the future.
